As a volunteer/participant of Always & Furever Midwest Animal Sanctuary (“Always & Furever”), I recognize and acknowledge that there are certain risks of physical injury and I agree to assume the full and entire risk of any and all injuries, including death, damages or loss which I may sustain as a result of volunteering / participating in any and all activities associated with Always & Furever. I understand that various activities involving dogs can be hazardous in nature involving the possibility of being bitten, scratched, jumped on, dragged or knocked over. I understand the risk of transferring disease-causing microorganisms during animal handling activities (ex: distemper, giardia, toxoplasmosis, and other parasitic or fungal conditions). I agree to take all precautions to avoid such transfer and the importance of having myself and my pets up to date on all current vaccinations. I understand that while Always & Furever will make every attempt for the safety of their volunteers, the possibility of serious accidents and/or injury can occur during participation in volunteer activities and I hereby agree to assume all risks of injury and to release and hold harmless Always & Furever and its directors, officers, and/or agents, heirs, successors, representations, assignees from and agents any and all liabilities, actions, claims, demands, costs, or expenses arising from or in any connected to with my participation as a volunteer, even in the case of negligence or misconduct of Always & Furever. I have voluntarily applied to participate in volunteer activities and hereby agree to assume all risks of injury and to release and hold harmless Always & Furever, its directors, officers and/or agents who through negligence or carelessness might otherwise be liable to me. I further agree to indemnify and to hold Always & Furever its directors, officers, and/or agents free and harmless from any loss, liability, damage, cost or expense which may incur as a result of any injury and/or property damage I may sustain while participating as a volunteer. I agree that all services I provide are charitable in nature and entirely voluntary, and there will be no compensation of any kind for the performance there for. Further I have been fully advised that as a nonemployee I am not covered by worker’s compensation and not covered under any medical insurance plan associated with my volunteer position with Always & Furever. I DO HEREBY FULLY RELEASE AND DISCHARGE Always & Furever, their officers, agents, servants, and employees, past or present, from any and all claims from injuries, including death, damages or loss which may accrue to me on account of my volunteering/participating with Always & Furever.

I further agree to indemnify and hold harmless and defend Always & Furever, their officers, agents, servants and employees, past or present, from any and all claims resulting from injuries including death, damages and loss sustained by me and arising out of, connected with, or in any way associated with volunteering / participating with Always & Furever.

I authorize Always & Furever volunteers to seek emergency medical treatment for me in case of accident, injury or illness.

I hereby grant Always & Furever permission to use my likeness in photograph / video or any photo/video that I upload to Always & Furever Facebook page, twitter, Instagram or any other form of social media, and/or in all of its publications, advertisements or collateral materials, including Always & Furever’s website without payment or consideration. I understand that I may not be reimbursed for any expense that I incur while volunteering, unless it has been pre-approved by Always & Furever.

I am aware that as a volunteer of Always & Furever, I agree to act responsibility at all times by maintaining a professional demeanor and understand the importance to protect the reputation of Always & Furever.
